B LEAV HOCKEY CAMP WAIVER AND RELEASE OF LIABILITY

By checking the box below, I acknowledge that I have read, understand, and agree to the terms outlined in this Waiver and Release of Liability for participation in B Leav Hockey Camp.

ASSUMPTION OF RISK

I acknowledge and understand that:

1. Hockey is a physically demanding sport involving physical contact, high-speed movement, and risks such as falls, collisions, or equipment-related injuries.

2. Despite on-ice supervision and safety measures, injuries—including but not limited to concussions, sprains, fractures, or more serious conditions—may occur.

3. There is absolutely NO supervision outside of scheduled on-ice activities. Parents/guardians are responsible for their child at all times when they are off the ice, including locker rooms, common areas, and any other parts of the facility.

4. I voluntarily allow my child to participate and assume all risks associated with the camp, including transportation to and from the facility.

WAIVER AND RELEASE OF LIABILITY

In consideration of my child’s participation in the B Leav Hockey Camp, I, on behalf of myself, my child, my heirs, executors, and assigns, hereby waive, release, and discharge the following parties from any and all claims, liabilities, damages, or causes of action arising from injury, illness, or loss incurred during participation in the camp:

• B Leav Hockey

• Brady Leavold

• Camp staff, coaches, volunteers, and affiliates

• Any facility hosting the hockey camp

This waiver applies regardless of whether the injury or damages arise from negligence or any other cause.

ZERO OFF-ICE SUPERVISION AGREEMENT

I acknowledge that B Leav Hockey Camp and its staff are only responsible for supervision during on-ice sessions.

• There is NO supervision before or after ice times, in the dressing rooms, lobby, parking lot, or anywhere else in the facility.

• It is my responsibility as a parent/guardian to ensure my child is supervised when not on the ice.

• I understand that B Leav Hockey and its staff are not liable for any incidents that may occur outside of scheduled on-ice instruction.

MEDICAL CONSENT

In the event of an emergency where I cannot be reached, I authorize B Leav Hockey staff, coaches, or medical personnel to provide necessary medical treatment to my child, including first aid, emergency transportation, or hospital care. I assume full responsibility for any medical expenses incurred.

CODE OF CONDUCT

I acknowledge that my child is expected to behave respectfully towards coaches, staff, and fellow participants. B Leav Hockey reserves the right to remove any participant from the camp due to inappropriate behavior, bullying, or non-compliance with camp rules, with no refund issued.

PHOTO/VIDEO RELEASE

I give permission for B Leav Hockey to use photos/videos of my child participating in the camp for promotional or marketing purposes, including social media and website content.

AGREEMENT

By checking the box below, I acknowledge that:
